Appendix F. Ordering T1/E1 PRI from the Carrier Questionnaire

The purpose of this questionnaire is to help you to order new T1/E1 lines or inform the carrier to make changes to the existing T1/E1 circuits to support deployment of the new IP Telephony (IPT) system. You can collect the information in this questionnaire from the customer and use it to place an order with the customer's carrier for new T1/E1 circuits or to make changes to the existing circuits. If you have T1/E1 circuits with multiple carriers, you have to place the order with each carrier.

Table F-1 provides the list of questions that carriers typically ask when someone is placing an order for T1/E1 PRI circuits. Some carriers might require additional information. Check with your customer's carrier and provide additional information if required.
